Communication and care coordination
With all members of the client’s care team: Doctors, Hospice nurses, therapists, social workers and clergy.
Attend medical appointments
This service benefits clients with memory issues whose family members are out of state or unavailable to accompany their loved one to appointments. Nurses will make sure that accurate medical information is shared with the provider and outcomes are communicated to the family designee so medical information doesn’t slip through the cracks.
